Getting your self-exclusion form submitted requires just a few steps, including verifying your identity via a photo of any government-issued ID (like a driver's license) to make sure states are able to process and apply your self-exclusion effectively.

Here is what you can expect:

1. Fill out self-exclusion form(s)

2. Get Connected with an Online Notary

If notarization is required, we connect you immediately to a free online notary via a partnership between NVSEP and Proof.

3. Digitally Sign the Document(s)

E-Signing the document(s) finalizes the process. NVSEP will then send them to the respective state agencies for procesing.
